I give consent for my son/daughter to participate in Bramley Phoenix FC events and agree to comply with the club and FA’s Parent/Carer Code of Conduct. I accept that it is my responsibility to inform the Club directly of any changes to the details recorded on this form. I agree to pay the fees for membership of the Club and understand that if payments are missed, my child’s membership may be cancelled.
