Before you complete any forms, please speak to your team manager and ensure you understand your Team Age Group.

Player Registration requirements

For New joiners only

1. Before registering with Bramley Phoenix, you are required to register both parent/guardian and child with the FA first. Please ensure all fields are completed as listed below, this form can be found at: FA form here

Ensure the following bits of information are completed:

i. Consent to play for the team
ii. Emergency contact for the child
iii. Full address and post code
iv. Medical details

For All players joining Bramley Phoenix FC

2. Once you have the child’s FA number you can register for the new season with Bramley Phoenix. You can access the registration form below:

3. After filling up the registration form; email a passport style photo and a valid form of ID (birth certificate/valid passport) to Aly.briscoe@bramleyphoenixfc.co.uk. Please add the child’s name and Team age group in the subject line.

4. Payment for registration and monthly subs is done via GoCardless, please complete the below mandate.

5. Once all forms and GoCardless link has been completed in full, your child’s league registration will be submitted.

6. Once the league has approved your child’s registration, your team manager will notify you.
